Your clinic has been part of a national program to improve how primary care and other services recognise and respond to family, domestic and sexual violence. Researchers from the Sax Institute are evaluating what has changed and whether it has made a difference to you.
Your feedback is completely voluntary and anonymous. It will not affect your care you receive from the service. You can stop at any time.
Can I take part?
- You have attended and received care from this service in the past 12 months
- You are aged 18 years or over
You do not need to have personally experienced family, domestic or sexual violence. Anyone who attended this service is welcome to share feedback.
How you can share
All options end with the same question: what is the most significant change you have noticed in your life as a result of the care you received?
