This review examines the current evidence base for clinical assessment tools that can indicate or predict the likely death of people within one year. For each tool identified, a description of the populations with whom, and the settings within which, the tool could be used is provided. The review also explores issues around implementation, including barriers and enablers, and an assessment of the tool’s potential applicability within the NSW context. While the review identified a broad range of tools, evidence around their implementation was limited, particularly in relation to their use in initiating end-of-life discussions with patients and their families/carers.
