This review examines optimal models of care for people with severe and enduring mental illness in hospital and community settings with a particular focus on young people with complex comorbidities. Optimal models delivered a combination of health, social and housing support services coordinated by a case manager. Essential components included: inpatient and community based rehabilitation etc.
